We offer same-day appointments for your convenience. For information or to schedule an appointment, please call our office at (931)967-7227 anytime. If during non-regular hours, one of our Patient Services Specialist will respond to your request within 24 business hours. The Appointment Request feature is for non-urgent appointments only. After we schedule your appointment, we will send you a confirmation e-mail.
Please, call us if have an urgent medical problem or you need to re-schedule an existing appointment. Contact our office at (931)967-7227.

PRESCRIPTION REQUEST
Prescriptions and refills for established patients (seen within the past one year) are routinely issued. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy. You can make your request by completing and submitting our Interactive Prescription Refill Request form below or by calling our office at (931)967-7227 during our regular office hours. For your safety, telephone authorizations for all controlled substances are prohibited. Federal law mandates prescriptions for these medications must be hand written by your doctor.
Please allow our staff two business days for all refill requests. This policy is for both online and phone requests.
CANCELATION POLICY
YOUR APPOINTMENT TIME IS RESERVED EXCLUSIVELY FOR YOU.
If you are unable to keep your appointment, kindly call us with at least 24 hours notice. This courtesy, on your part, will make it possible to give your appointment to another person. In the event you cancel within 24 hours of your appointment, we will charge a cancellation fee of
$25 for Office Visits
$50 for Testing
$150 for Nuclear Stress Tests.
Kindly note the higher fee for office procedures. Testing slots are especially difficult to fill on short notice because of required insurance authorizations and regulatory requirements. Additionally, nuclear stress tests require us to purchase a medication specifically for you and your designated appointment time. If we are unable to cancel this medication order, the cost will be billed directly to the patient and is not billable to insurance.
